Application for the PhD Degree

This form is to be used by PhD students who are about to finish their research and are preparing to defend their PhD dissertation. This form is to be submitted to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; either RH412A or RH216) at the start of the semester in which the student is planning on defending his/her dissertation.

Request for Appointment or Reconstitution of PhD Guidance Committee

This form is to be used by PhD students. Within six (6) months of passing the PhD qualifying examination(s), the student and the dissertation adviser must form or appoint a dissertation guidance committee. Use this form to identify the members of such committee. In addition, two (2) weeks prior to defending the final PhD dissertation, the student will be required to reconstitute the guidance committee. This form is to be submitted to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; either RH412A or RH216).

Request to Schedule Dissertation Defense for the PhD Degree

This form is to be used by PhD students to schedule their final PhD dissertation defense. This form is to be submitted to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; either RH412A or RH216) no later than two (2) weeks prior to the scheduled date.

Transfer Credit Request for MS and PhD Graduate Students

This form is to be used by graduate (MS and PhD) students wishing to transfer credits from another institution to satisfy degree requirements at NYU-Poly. The requested transfer credits must have been completed prior to being admitted to NYU-Poly. After obtaining all required approvals, student must submit the form with a copy of the unofficial transcript to Prof. Jose Ulerio (Office of Associate Provost for Graduate Academics; either RH412A or RH216). Students must first complete a minimum of nine (9) credits at NYU-Poly and must be in good academic standing (GPA >=3.0) prior to requesting transfer credits.

Graduate GPA Deficiency Acknowledgement

This form is to be used by graduate (MS and PhD) students wishing to have a GPA deficiency hold removed. Students are issued deficiency holds when their CUM GPA does not meet the minimum University requirements as outlined in the table below:

Failure to meet these requirements will result in disqualification from further studies at the NYU-Poly.

After obtaining approval from your academic adviser student should submit form tothe office of the Associate Provost for Graduate Academics (RH 216).

Application for Appeal of Academic Disqualification

This form is to be used by graduate (MS and PhD) students appealing academic disqualification from the University.   Students must write an appeal letter which must be submitted with the completed and signed application along with a copy of the disqualification letter to the office of the Associate Provost for Graduate Academics (RH 216).

ACP Probation

This form is to be used by graduate (MS and PhD) students placed on ACP Probation hold. Students wishing to have the hold removed must complete this form, develop a plan that addresses the GPA deficiency, and obtain the signature of the academic adviser before submitting to the office of the Associate Provost for Graduate Academics (RH 216).

Request for Adjustment of Degree Requirements

This form is to be used by graduate (MS and PhD) students requesting adjustments to their degree requirements. Approval must be obtained from both their academic adviser and academic department head. After approvals students must submit form to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).

Request for Extension of Time to Complete Degree Requirements

The program for graduate certificate must be completed within 3 years. The MS degree programs must be completed within 5 years of starting the program at NYU-Poly. The PhD program must be completed in 6 years for full-time students and 12 years for part-time students from the time of admission to graduate work at Poly-NYU. These time limits include approved leaves of absence.

Students may use this form to request an extension exceeding these time limits. For more information regarding time extensions refer to the University catalog.

Once form is completed student must submit to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).

Request to Declare or Change Major or Concentration or Matriculation Status

Graduate (MS and PhD) students may use this form to request to declare or change major or concentration or matriculation status. This form cannot be used to change from a Certificate to an MS program, nor from an MS to a PhD program.  In both cases the student must formally apply to the program.  Once form is completed and all necessary approvals are obtained, student must submit form to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).

08 OGA eforms Request to Declare or Change Major or Concentration or Matriculation Status


Application to a Certificate Program and Application for a NYS-Registered Certificate

Graduate students may use form 09 to apply for a New York State-registered certificate. Once the form is completed and all necessary approvals are obtain, student must submit to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).  Form 09 must be accompanied by a copy of the student's unofficial transcript clearly showing the courses which are being used to satisfy the certificate requirements.  For more information on the requirements of the certificates consult the latest University catalog and/or your academic faculty adviser.  In addition to form 09 students must formally apply to the certificate program and pay a $75 application fee.  You may apply to the certificate program by completing the Application to a Certificate Program (see form 09a below). DEADLINE FOR APPLYING TO A CERTIFICATE PROGRAM AS WELL AS APPLYING FOR A CERTIFICATE HAS BEEN EXTENDED TO FRIDAY, DECEMBER 2, 2011.

Application for Polytechnic Graduate Students to Take Classes at NYU

MS and PhD students may use this form to request approval to take courses at NYU. Once form is completed and all necessary approvals are obtain, student must submit form to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).  Students must have a minimum CUM GPA of 3.3 (B+) or higher at the time the application is submitted for approval.  For more information refer to the Policy on taking classes at NYU above.  DEADLINE FOR SUBMISSION IS FRIDAY, DECEMBER 23, 2011.

Request for GPA Restart

MS graduate students may use this form to request a GPA restart. Once form is completed and all necessary approvals are obtain, student must submit to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics; RH412A or RH216).  You should consult with your academic adviser regarding the consequences of requesting a GPA Restart.
