Application for the PhD Degree

This form is to be used by PhD students who are about to finish their research work and are preparing to defend their PhD dissertation. This form is to be submitted to Prof. Jose Ulerio located in RH412A at the start of the semester in which the student is planning on defending his/her dissertation.

Request for Appointment or Reconstitution of PhD Guidance Committee

This form is to be used by PhD students. Within six (6) months of passing the PhD qualifying examination(s), the student and the dissertation adviser must form or appoint a dissertation guidance committee. Use this form to identify the members of such committee. In addition, two (2) weeks prior to defending the final PhD dissertation, the student will be required to reconstitute the guidance committee. This form is to be submitted to Prof. Jose Ulerio located in RH412A.

Request to Schedule Dissertation Defense for the PhD Degree

This form is to be used by PhD students to schedule their final PhD dissertation defense. This form is to be submitted to Prof. Jose Ulerio located in RH412A no later than two (2) weeks prior to the scheduled date.

Transfer Credit Request for MS and PhD Graduate Students

This form is to be used by graduate (MS and PhD) students wishing to transfer credits from another institution to satisfy degree requirements at NYU-Poly. The requested transfer credits must have been completed prior to being admitted to NYU-Poly. After obtaining all required approvals, student must submit the form with a copy of the unofficial transcript to Prof. Jose Ulerio located in RH412A or the Office of the Associate Provost for Graduate Academics located in RH216.

Graduate Acknowledgment of Academic Probation and Potential Disqualification Form

This form is to be used by graduate (MS and PhD) students whose CGPA has fallen below the University requirement of 3.0. 

All graduate students are expected to maintain a 3.0 CGPA during their graduate career.  Academic performance will be evaluated twice per academic year, at the end of each fall and spring term.   Students who do not have a CGPA of 3.0 or above at the time of evaluation will be automatically placed on academic probation for the subsequent term.

Students are permitted a maximum of two (2) semesters of academic probation for the entirety of their graduate program.  The semesters do not have to be sequential in order to count towards this maximum limit.

Failure to achieve a 3.0 CGPA by the end of the second probationary semester, or falling below 3.0 for a third semester, will result in permanent disqualification from NYU-Poly.  Disqualified students will not be considered for readmission.

Please note that the 3.0 CGPA requirement is exclusive of any Incomplete or Satisfactory grade(s).

After obtaining a signature from the academic adviser, probationary students must submit this form to the Office of the Associate Provost for Graduate Academics located in RH 216.

Request for Adjustment of Degree Requirements

This form is to be used by graduate (MS and PhD) students requesting adjustments to their degree requirements. Approval must be obtained from both their academic adviser and academic department head. After obtaining the required approvals students must submit form to the Office of the Associate Provost for Graduate Academics located in RH216.

Request for Extension of Time to Complete Degree Requirements

The program for graduate certificate must be completed within 3 years. The MS degree programs must be completed within 5 years of starting the program at NYU-Poly. The PhD program must be completed in 6 years for full-time students and 12 years for part-time students from the time of admission to graduate work at Poly-NYU. These time limits include approved leaves of absence.

Students may use this form to request an extension exceeding these time limits. For more information regarding time extensions refer to the University catalog.

Once form is completed student must submit to the office of the Associate Provost for Graduate Academics (RH216).

Request to Declare or Change Major or Concentration or Matriculation Status

Graduate (MS and PhD) students may use this form to request to declare or change major or concentration or matriculation status. This form cannot be used to change from a Certificate to an MS program, nor from an MS to a PhD program.  In both cases the student must formally apply to the program.  Once form is completed and all necessary approvals are obtained (If you are an international student you must also obtain approval from OISS), student must submit form to the Office of the Associate Provost for Graduate Academics located in RH216.

Application TO a NYS-Registered Certificate Program and Application FOR a NYS-Registered Certificate

Graduate students may APPLY TO a certificate program ONLY during their first semester of graduate studies, OR prior to taking any classes that would satisfy the certificate program.  You may apply by completing the Application to a Certificate Program (see form 09a below), having the certificate program advisor approve, paying the $75 application fee to Student Accounts, and submitting the form to Prof. Jose Ulerio in RH412A.  The deadline for applying TO a certificate program is 5:00PM on Monday, April 15, 2013.

Once you've completed or are about to complete the courses that satisfy the certificate program you may use the Application for a Certificatef (see form 09 below) to APPLY FOR the certificate. Once the form is completed and all necessary approvals are obtain, form must be submitted to Prof. Jose Ulerio in RH412A.  Form 09 must be accompanied by a copy of the student's unofficial transcript highlighting the courses which are being used to satisfy the certificate requirements.  For more information on the requirements of the certificates consult the latest University catalog and/or your academic faculty adviser.   You may apply FOR the certificate through Self Service in PeopleSoft – this deadline is Friday, February 22, 2013.  After you've applied through Self Service you will need to submit the completed form to your academic adviser for approval before dropping off at Prof. Ulerio's office located in RH412A.

09 OGA eforms Application for a Certificate


Application for NYU-POLY Graduate Students to Take Classes at NYU

MS and PhD students may use this form to request approval to take courses at other NYU schools.  Students must have a minimum CUM GPA of 3.3 (B+) or higher at the time the application is submitted for approval.  For more information refer to the Policy on taking classes at NYU above.

Steps to taking classes at NYU:

  1. Obtain Information on class offerings through NYU's Albert System.
  2. Get approval from your academic advisor regarding your class selection.  Class must be able to satisfy degree requirements.
  3. Obtain permission from Graduate Academics (see Prof. Jose Ulerio in RH412A).
  4. Obtain email or signature approval from instructor of course you plan to take.
  5. Visit NYU school to get registered.
  6. Upon returning to NYU-Poly have OISS approve (International students only) before returning original application form back to Prof. Ulerio (RH412A) for final processing and registration on the NYU-Poly side.

Please note that permission for mathematical finance courses at Courant are limited and will require an additional level of approval from Prof. Barry Blecherman.  Permission for these courses are issued based on GPA and senior status in your studies.  All applications for these courses must be submitted to Ms. Zahra Patterson (RH519C) by date to be announced.

THE DEADLINE FOR ALL OTHER COURSES IS 5:00PM ON Wednesday, May 22, 2013.  Registration at NYU will take place during the first week of classes in the fall.

Request for GPA Restart

MS graduate students may use this form to request a GPA restart. Once form is completed and all necessary approvals are obtain, student must submit to Prof. Jose Ulerio (Office of the Associate Provost for Graduate Academics lcoated in RH216).  You should consult with your academic adviser regarding the consequences of requesting a GPA Restart.
