TO REPRINT YOUR INSURANCE ID CARD
The Enrollment and Waive period for Fall 2009 will begin in July.
All Students (Fall 2009)
All Polytechnic students (Undergraduate and Graduate) for the Fall 2009 semester who are enrolled in 9 or more credits will automatically be enrolled in the Polytechnic sponsored health insurance plan. The fee for this plan will be placed on your tuition bill. Coverage will be from August 24, 2009 and end on August 24, 2010. The semester fee is $345 for Fall 2009 per student.
The waiver period (new domestic students only) for Fall 2009 is October 2, 2009. If you have questions regarding your enrollment, your card or the plan, please contact Malinda Townsend of Collegiate Insurance Resources at 1-800-322-9901, select option 6 and ask for Ms. Townsend.
Remember your health insurance is provided by Combined Insurance and your Pharmacy plan is provided by Medco.
International students
You are REQUIRED to be enrolled in the Polytechnic sponsored plan as part of your enrollment at NYU-Poly regardless the number of credits you are enrolled in. If you are an International Student at NYU-Poly, you MUST print your card by October 2, 2009. Coverage will be from August 24, 2009 and end on August 24, 2010. To print your card, scroll to the top of this page and click the blue "International Students Print Card".
Domestic Student Enrollment
You MUST complete the enrollment information by October 2, 2009 for Fall 2009 coverage. Coverage will be from August 24, 2009 and end on August 24, 2010. The sooner you enroll the sooner you will have your coverage information. To Enroll, scroll to the top of this page and click the green "DOMESTIC STUDENTS ENROLL" button.
Domestic Student Waiver for Students with Alternative Health Insurance
If you have private insurance with comparable coverage, you have until October 2, 2009 to waive the Polytechnic plan. Click the red "DOMESTIC WAIVE" button to apply for a waiver (applying for a waiver does not automatically waive you out of the plan). You will be contacted by the insurance carrier if there are questions regarding your waiver. If you do not waive by the October 2, 2009deadline, you will be responsible for the fee.
Continuing Students that were Part-Time and are now Full-time
All NYU-Poly students (Undergraduate and Graduate) who were part-time in the Spring 2009 semester and are now enrolled in 9 or more credits for the Fall 2009 semester will automatically be enrolled in the Polytechnic sponsored health insurance plan. The fee for this plan will be placed on your tuition bill. Coverage will be from August 24, 2009 and end on August 24, 2010. The semester fee is $345 for Fall 2009 per student. Domestic student may waive if they have comparable coverage.
Part Time Students
For students enrolled in fewer than nine (9) credits, Polytechnic provided insurance is optional. If you do not want coverage, you do not have to do anything. The fee for Fall 2009 part-time students is $362 for coverage from August 24, 2009 to January 19, 2010.

How to Print Out Your Insurance Card
Please note, you must be enrolled before you can print a card.
You need to have you health insurance membership # to print your card. If you do not have this number, please contact Malinda Townsend of Collegiate Insurance Resources at 1-800-322-9901 Option 6 for assistance.

New Discount Program Available
A new voluntary Discount Program is available to all Polytechnic students. It provides students, and those within their household, with discounts on the following services: vision care, dental care, prescription drugs, chiropractic care, alternative medicine, hearing services, vitamins and nutritional supplements, and a national fitness program. Discounts range from 10-50% depending upon the services utilized. The annual cost is $29. Again, this is a discount program and is not health insurance.
