The enrollment period for Fall 2008 is now open. If you have questions regarding your enrollment, waiving out (domestic students only), your card or the plan please contact Malinda Townsend of Collegiate Insurance Resources at 1-800-322-9901 for assistance.
All Students (Fall 2008)
All Polytechnic students (Undergraduate and Graduate) for the Fall 2008 semester who are enrolled in 9 or more credits will automatically be enrolled in the university-sponsored health insurance plan. The fee for this plan will be placed on your tuition bill. Coverage will be from August 24, 2008 and end on August 24, 2009. The annual fee is $565 per student. This will be charged semesterly ($283 for Fall 2008; $282 for Spring 2009).
International students
You are REQUIRED to be enrolled in the university-sponsored plan as part of your enrollment at Polytechnic. If you are an International Student at Polytechnic, you MUST print your card by October 3, 2008. Coverage will begin on August 24, 2008 and end on August 24, 2009. To print your card, scroll to the top of this page and click the blue "INTERNATIONAL STUDENTS PRINT CARD" button.
Domestic Student Enrollment
You MUST complete the enrollment information by October 3, 2008 for fall 2008 coverage. Coverage will begin on August 24, 2008 and end on August 24, 2009. The sooner you enroll the sooner you will have your coverage information. To Enroll, scroll to the top of this page and click the green "DOMESTIC STUDENTS ENROLL" button.
How to Waive for Students with Alternative Health Insurance
(Domestic students only)
If you have private insurance with comparable coverage, you have until October 3, 2008 to waive the university plan. Click the red "DOMESTIC WAIVE" button to apply for a waiver (applying for a waiver does not automatically waive you out of the plan). You will be contacted by the insurance carrier if there are questions regarding your waiver.
Part Time Students
For students enrolled in fewer than nine credits, university-provided insurance is optional. If you do not want coverage, you do not have to do anything. The fee for Fall 2008 part-time students is $300 for coverage from August 24, 2008 to January 20, 2009.

How to Print Out Your Insurance Card
Please note, you must be enrolled before you can print a card.

- Click on “ACI Claim Status”
- Click on “Insured”
- Fill out the form
- Select the reprint Insurance Card option
New Discount Program Available
A new voluntary Discount Program is available to all Polytechnic students. It provides students, and those within their household, with discounts on the following services: vision care, dental care, prescription drugs, chiropractic care, alternative medicine, hearing services, vitamins and nutritional supplements, and a national fitness program. Discounts range from 10-50% depending upon the services utilized. The annual cost is $29. Again, this is a discount program and is not health insurance.
