Enroll into or drop from an NYU-Poly course when you cannot do so online for special circumstances. This form may be used for audit requests (graduate students) and variable credit course permission (undergraduate and graduate students).
Full-time undergraduate students may register for 12-21 credits (all credits in excess of 20 are charged at the per credit rate). Graduate students may register for a maximum of 12 credits.
Credit ‘overloads’ are approved on a case-by-case basis and dependent upon a student’s academic performance. Students who wish to appeal for overload must submit a Credit Limit Overload Form to the Office of Undergraduate/Graduate Academics. The form must be signed first by the department adviser. If ‘overload’ permission is granted, the completed form must be submitted to the Registrar’s Office for processing. The Registrar’s Office will adjust the student’s maximum credit load, allowing the student to enroll for the approved credit amount online via PeopleSoft Self-Service.
Appeal your academic disqualification from NYU-Poly. Your appeal must include an essay explaining the reason(s) for the appeal and must be approved by your academic adviser and the Director of Academic Affairs.
Waive your FERPA rights, which will allow third parties to see your education records. You will be able to indicate which third parties can view the records.

Learn about the process of taking a course at another school of NYU and to get permission to complete a course. You must receive permission from your adviser and obtain equivalent course information from the corresponding NYU-Poly Department.
NOTE: Spring 2013 registration at NYU will open December 10, 2012. The last day to submit a request form is December 12, 2012 (last day of undergraduate classes).
Get permission to study abroad at one of NYU's Global Sites. You must receive permission from your adviser and obtain equivalent course information from the corresponding NYU-Poly Department(s).
Get permission to take a class at another college or university for transfer credit. You must complete this form before you begin the course by receiving permission from your adviser and obtaining equivalent course information from the corresponding NYU-Poly Department.
Request a particular course of action to complete a stated degree requirement, for example by replacing one required course with another similar course. In consultation with your academic adviser, you will complete the form which will then need to be approved by the Dean of Undergraduate Academics.
Request full-time status while you are taking a part-time course load. This request should only be made for very specific circumstances, e.g., if you have a co-op assignment or are completing thesis work. You should consult with your academic adviser to determine if this is an appropriate course of action and complete the form for approval of both the adviser and the Dean of Undergraduate Academics
Enroll in RE4440 – Maintenance of Study. If you have finished your courses, but still need time to complete other academic requirements, e.g., theses, projects, or incomplete grades, you may want to enroll in RE4440 – Maintenance of Study in order to maintain your status as a matriculated student at NYU-Poly, which is required to graduate from the Institute. To enroll, you will write an essay explaining why you were unable to complete your work in the time allotted. Your request must be approved by your academic adviser and the Dean of Undergraduate Academics.
Declare or change a major, or add a minor to your degree program.
Learn about the process of declaring a cross-school minor at another school of NYU. You must receive permission from your adviser and meet other requirements to declare a minor in accordance with the cross-school minor policies.
Reserve a room for a special event. This form should NOT be used final exams or semester-long events.
As per NYU-Poly policy, enrolling for two courses with overlapping meeting times is prohibited. Leaving a class early or arriving late is disruptive to the instructor and other students in the class. Such time conflicts also jeopardize a student’s academic success in a class. This form is only to be used if a student, with the agreement of their adviser and course instructors, believes that extenuating circumstances warrant an exception to this rule. Please attach a Course Enrollment & Drop form and submit BOTH forms to the Registrar’s Office for consideration.
Get an extension to complete your degree beyond the 8-year time limit. If you do not complete your degree requirements within the 8-year time limit, you must complete this form in order to be allowed to finish your degree at NYU-Poly. You must write an essay explaining why you were not able to finish your degree within the allotted time. Your academic adviser will review the form and fill out the course(s) that you must complete in order to graduate. The Dean of Undergraduate Academics will make the final approval.

Request a copy of your transcript. There is a charge of $5 for each transcript.
Withdrawal from a course if you cannot do so online. Approval is needed from your adviser. (Note: to withdrawal from the Institute use the Leave of Absence/Total Withdrawal form.)
